Who Needs a SAP Evaluation?

DOT SAP evaluations are required for individuals working in safety-sensitive positions regulated by the U.S. Department of Transportation who have:

• Tested positive for drugs or alcohol

• Refused a DOT drug or alcohol test

• Violated DOT drug and alcohol regulations

These evaluations are part of the return-to-duty process required before resuming safety-sensitive job functions.

We provide professional SAP evaluations for clients in Iowa, Nebraska, and South Dakota via telehealth.

In-Person Services

In addition to telehealth services, in-person SAP evaluations are available at our Sioux City, Iowa office for clients who prefer face-to-face appointments. Evaluations are conducted by a DOT-qualified Substance Abuse Professional (SAP) and licensed mental health counselor.

Fees & Important Information

SAP Evaluation Package: $500
Includes initial evaluation, follow-up evaluation, and all required documentation for the return-to-duty process.

Evaluation Time:
Initial evaluations typically take 60–90 minutes. Follow-up evaluations are shorter and scheduled after completion of any required recommendations.

Turnaround Time:
Evaluation reports are typically completed within 24–48 hours after the initial evaluation, provided all required information has been received.

Expedited Service:
Expedited appointments and faster turnaround times may be available upon request.

Payment:
Payment is required prior to the initial evaluation. Insurance is not accepted.

Important:
DOT SAP services are professional evaluations conducted in accordance with federal guidelines and are not therapy or treatment services. Recommendations may include education or treatment with an external provider, if required.

This service is for employment-related DOT requirements and is not for court-ordered DUI/OWI evaluations.
